OMC Offering Free Athletic Physicals For Area Students July 23

WEST PLAINS, Mo. — Ozarks Medical Center will offer free athletic physicals for area student-athletes from 12-7 pm Thursday, July 23 at the OMC Parkway Center.

 

Local physicians, family nurse practitioners, and advanced practice nurses will conduct physicals for student-athletes entering 7th through 12th grades attending local area schools.

 

To follow social distancing guidelines, participants are encouraged to arrive during specific time slots, determined by their school or high school team. OPEN times are for teams outside of the West Plains R-7 and Rural School areas.

 

Times and locations are as follows:

12 – 1 p.m. Softball, Tennis Fairview Tennis, Golf
1 – 2 p.m. Cross Country Track Glenwood Cross Country, Track
2 – 3 p.m. Volleyball, Wrestling Howell Valley Soccer, Swim
3 – 4 p.m. Basketball, Cheer Junction Hill Basketball, Wrestling
4 – 5 p.m. Soccer, Swim Richards Football
5 – 6 p.m. Golf/OPEN WP Middle School Baseball/OPEN
6 – 7 p.m. OPEN OPEN OPEN

 

Student-Athletes are asked to check-in at the tents located near the assigned Conference Room. Temperatures and COVID-19 screening questions will be asked upon arrival. Once checked in, students will be directed to the appropriate location to complete their athletic physical.

 

All students will be required to submit a signed consent/medical history with concussion form and current insurance information from a parent or legal guardian at the time of the physicals. Consent forms may be picked up between 8 a.m. to 4 p.m. Monday through Friday, at the West Plains High School office; at OMC Education Services in the Parkway Center in West Plains; or OMC Rehabilitation Services at the Shaw Medical Building, 1111 Kentucky Ave. Forms are also available online https://www.ozarksmedicalcenter.com/documents/events/New-Physical-Form-2year.pdf
